RESUMO

The concept of "syndemics" is getting more and more popularity in scientific journals, especially since the end of the first decade of the current century. It relates to the dynamic interaction of synchronous or sequential diseases (whether communicable or not, also including mental diseases), with social and environmental factors, resulting at the end in a worse global outcome. The rise of publications in peer reviewed journals is exponential, especially in the last ten years. Born after another - forgotten - epidemy, the one related to HIV (AIDS), this concept is more than ever cited within the frame of the still ongoing COVID-19 pandemics. We will highlight this concept through a couple of selected examples related to infectious diseases. The recognition of an extensive intertwining allows to change in depth the way we approach health care efficiency, both at an individual as well as a societal level.


Le concept de «syndémie¼ - l'interaction entre maladies co-existantes ou séquentielles (transmissibles, non transmissibles, et maladies mentales), avec des phénomènes sociaux et environnementaux qui amplifient les effets négatifs de cette interaction - fait de plus en plus le sujet de publications dans des journaux internationaux, particulièrement ces dernières années. La croissance d'articles est effectivement exponentielle en particulier depuis la fin de la première décade du 21ème siècle. Né dans les suites d'une autre épidémie - celle du SIDA (taxée d'ailleurs de pandémie oubliée) - ce terme est devenu de plus en plus populaire. Il l'est encore plus aujourd'hui, depuis l'avènement de la crise sanitaire liée à une autre pandémie, celle de la COVID-19. Les liens et interactions multiples entre maladies et facteurs «externes¼ seront illustrés à l'aide de quelques exemples relatifs aux maladies infectieuses. Le fait même de reconnaître un tel entrelacement permet d'évoquer une approche différente afin d'améliorer l'efficience de la prise en charge de la santé d'une population et d'un individu.

Health Literacy (HL) is defined as «the knowledge, motivation, and competences to access, understand, appraise and apply health information in order to make judgments and take decisions in everyday life concerning health care, disease prevention and health promotion to maintain or improve quality of life¼. This concept begins to be recognized as a priority area for action at political level in Belgium. Indeed, a limited HL may affect health by leading, for example, with poorer management of chronic diseases, more hospital admissions and premature deaths. This paper addresses the question of HL evaluation, improvement interventions as well as the many challenges that remain in this area. HL seems fundamental to the development of better health management. It would allow patients to play a more active role in health care, to involve all health stakeholders, and to contribute to a more sustainable health system. Improving HL could allow better equal access to health care.e.


La Health Literacy (HL) se définit comme 'la connaissance, la motivation et les compétences des individus à accéder, comprendre, évaluer et appliquer l'information en matière de santé en vue de porter des jugements et de prendre des décisions de tous les jours en ce qui concerne la santé, la prévention des maladies et la promotion de la santé, de manière à maintenir ou améliorer la qualité de vie'. Au niveau politique belge, ce concept commence à être reconnu comme étant un domaine d'action prioritaire. En effet, une HL limitée peut affecter la santé en conduisant, par exemple, à une moins bonne gestion des maladies chroniques, à davantage d'hospitalisations et de décès prématurés. Cet article aborde la question de l'évaluation de la HL, des interventions pour l'améliorer ainsi que les nombreux défis qui restent à relever dans ce domaine. La HL semblerait fondamentale pour le développement d'une meilleure gestion de la santé. Elle permettrait au patient de jouer un rôle plus actif dans la prise en charge de sa santé, de responsabiliser tous les acteurs de la santé, et de contribuer à un système de santé plus viable. Améliorer la HL devrait permettre une meilleure égalité d'accès aux soins de santé.

RESUMO

Convergence between information and communication technology and recent developments in medical care will totally change the health care sector. The way we perform diagnosis, treatment and follow-up will undergo disruptive changes in a very near future. We intend to highlight this statement by a limited selection of examples of radical innovations, especially in the field of oncology. To be totally disruptive and to illustrate the concept of "lateral power" - especially cognitive distribution - the list of references is only made up of internet links. Anyone - patients included - can easily and instantly access to this information everywhere.


La convergence entre les TIC (Technologies de l'Information et de la Communication) et les développements technologiques en médecine vont totalement changer le paysage en matière de prise en charge de la maladie, mais aussi en matière de prévention. Il y a fort à parier que nos méthodes de diagnostic, de traitement et de suivi vont subir des changements «disruptifs¼ et ce, à court terme. Nous mettrons ce propos en évidence dans le domaine de l'oncologie. Pour illustrer les «forces transversales¼ (en particulier la connaissance distribuée) qui poussent ces processus de changements, et pour être disruptif jusqu'au bout, nous nous référerons uniquement à des liens Internet. Tout un chacun ­ en ce compris les patients et leurs familles ­ peut aisément accéder à cette information de façon immédiate et ubiquitaire.

The history of the treatment of vestibular schwannoma (VS) clearly shows a shift from microsurgery towards radiotherapy as "standard therapy". The microsurgical approach obviously yields excellent tumour control, but is hampered by a high morbidity rate, especially at the level of cranial nerves. The literature reports a normal facial nerve function in only 60% of patients surgically treated for VS with a maximum diameter of 25 mm. Hearing preservation is only around 15% for tumours > 15 mm and around 50% for tumours less than 15 mm. Progress in techniques and computer sciences in the field of radiotherapy provides the opportunity to reach high levels of tumour control (92 - 98%), keeping cranial nerve morbidity on a very low level (< 2%). Hearing capacity and serviceable hearing can be maintained in about 70% of cases. Based on these results, one can assume that radiation therapy should be used upfront for those benign tumours.

RESUMO

Conventional radiotherapy is known to be an effective treatment approach even for "benign" pathologies. However, this kind of treatment yields a high potential for side effects. The Cyberknife, a robotic stereotactic radiotherapy device, enables to offset a large proportion of the disadvantages encountered with conventional radiotherapy essentially through the high precision of dose administration and sparing of healthy tissues. Therefore, it seems to be a treatment of choice in the approach of some benign intracranial diseases. We review published data on indications and outcome of Cyberknife for intracranial "non-malignant" disease.

RESUMO

The aim of adjuvant hormone therapy for breast cancer is to reach, in daily practice, an efficacy similar to that obtained in clinical trials. In spite of the demonstrated efficacy of hormone therapy, compliance represents a major challenge and a multidimensional problem. A better understanding of the reasons underlying non-compliance would help identify the patients at higher risk and would permit the implementation of strategies to improve compliance to adjuvant hormone therapy. With this in mind, we undertook a review of the recent literature on the topic (Pub Med 2003-2009).
